The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H + H) is responsible for the delivery of medical care, mental health care and discharge planning in the jail system. CHS is a national leader in the delivery of healthcare to the criminal-justice involved population and aspires to the highest standards of care. CHS care includes a comprehensive history and physical at the point of admission, robust screening efforts (HIV, hepatitis C, STDs, TB, etc.) and the management of chronic and acute illness. CHS views incarceration as a crucial opportunity to provide care to an underserved population and improve the population health of the communities to which our patients return. CHS is committed to continuously improving the delivery of care in the security setting, using the system-wide Electronic Health Record as a tool to promote patient safety and population health. Responsibilities

Schedule: Full-Time, Monday - Friday 8am-4pm (CDU-WF)

 

Under the supervision of the Mental Health Unit Chief, the Mental Health Clerk will perform various administrative and clerical tasks to support the facility mental health operations. This includes managing the appointment lists to ensure proper scheduling of mental health care visits and assisting with the maintenance of patient databases and spreadsheets during the day tour (8am- 4pm), Monday through Friday.

 

  • Assist in answering incoming calls and directing them to appropriate staff.
  • Maintain patient census database (when applicable)
  • Reconcile and report MO Bed space (when applicable)
  • Processing and management of MO round sheets. This includes tracking/scanning and inputting into electronic record or databases (when applicable)
  • Utilize the Inmate Inquiry System (IIS) in order to generate the lists of patients that were transferred into and out of the facility, and discharged in order to update patient tracking database.
  • Generate daily suicide watch list and disseminate to appropriate parties
  • Assign “M” designation to newly enrolled patients via current procedures
  • Provide real-time scheduling support and assist with timeliness completion
  • Obtain/manage collateral records from outside institutions and agencies (e.g. hospital discharge summaries, psychological testing reports, confirmation of housing placement) as requested by mental health clinical staff member. This includes serving as the point of contact for outside medical records regarding collateral information, informing requesting clinical staff that records have arrived, and scanning all information received into the chart
  • Chaperone telehealth encounters as identified by unit chief
  • Maintain department equipment and reorder supplies
  • Handle sensitive information in a confidential manner
  • Perform additional related duties as assigned by supervisor

 

 

Qualifications

  • Associate's Degree of equivalent education required
  • Knowledge of relevant software applications including MS office, MS Word, Excel
  • Proficient in the use of email and internet
  • Excellent communication skills - verbal and written
  • Strong data management
  • Exceptional time management skills and ability to prioritize work
  • Experience in maintaining patient confidentiality
