NYC Health + Hospitals/Harlem, located on 135th Street in Manhattan, is a designated Level 1 Trauma Center and is recognized for providing centers of excellence in the areas of Bariatric Surgery, Breast Imaging and Sexual Assault Forensics. Our teams provide services for nearly 213,000 outpatient visits, more than 83,000 emergency room visits and over 13,000 inpatient admissions annually. The hospital maintains an academic affiliation with Columbia University Medical Center.

Responsibilities

GRANT FUNDED - HARLEM HOSPITAL CENTER, DEPARTMENT OF PEDIATRICS, FAMILY CARE CENTER is a hospital based multidisciplinary program that provides primarily outpatient psychosocial services- psychotherapy, case management, discharge planning, family assessment, interagency coordination, and supportive services to HIV/AIDS infected adults, adolescents/young adults.

 

The program is currently recruiting for a full-time Licensed Mental Health Clinician (LMHC). Responsibilities will include comprehensive psychosocial assessments and making the appropriate referrals. Providing individual, group, and family psychotherapy will be important.

  • Provide mental health monitoring, individual therapy and treatment adherence counseling to adults, adolescent/young adults, and transitioned young adults; Also provide consultation to PCPs and conduct evaluations for mothers and new adult referrals;
  • Attend weekly adolescent/young adult clinics, monitoring” caseload of stable adolescent/young adult cases during clinics;
  • Co-facilitate support group with the mental health staff member; Participate in care coordination meetings for comprehensive treatment planning and interdisciplinary collaboration.
  • Participate in the adolescent and young adult Community Advisory Board; Attend Pre-transition Drop-In Programming; Develop and maintain active contact with community-based resources to assist clients.
  • Participate in quality improvement activities for the mental health program.
  • Improve proficiency in managing psychiatric crises with severe and persistent mentally ill.
  • Participate in group supervision with other group leaders and clinical staff. Work closely as a member of an interdisciplinary mental health and medical team.
  • Perform other related duties as assigned

 

Qualifications

  • A master's degree from an accredited school of social work and at least two years of experience in providing case management services and psychotherapy/supportive counseling.
  • Must have a current license - Social Work (LMSW) issued by New York State Department of Education.
  • Should be computer literate with familiarity with Microsoft Office suite programs and data management.
  • Proof of Covid-19 vaccination is required prior to employment
