Primary Care Physician

Physician Affiliate Group of New York (PAGNY) is adding a Primary Care Physician to the group at NYC Health + Hospitals/Harlem. The Adult Clinic at NYC Health + Hospitals/Harlem is designed to promote wellness and prevent disease and disability. Our physicians are nationally recognized experts in their fields. Special programs/support groups for those with chronic illness are held to provide additional help for diabetes, cancer, amputee education & Alzheimer’s/Dementia. The hospital maintains an academic affiliation with Columbia University Medical Center.

• The service at Harlem Hospital is an academic practice sponsored by the Columbia University Medical Centeroutpatient care, clinical supervision of fellows, residents and medical students
• Practice has 15 - 20 patients daily - time with patient/quality patient care is promoted 
• Weekly team meeting to encourage collaboration and new ideas 
• The practice is a research site for diabetes & fully staffed with support staff such as social workers, clinical pharmacists & dietician
