The Stroke Program Coordinator oversees and coordinates care of stroke patients through the continuum of Harlem Hospital's Stroke Program. This position is responsible for maintaining quality standards of care, professional documentation including data entry into "Get with the Guidelines", supporting current policies and procedures, and program certification.

The Stroke Program Coordinator is expected to deliver care to the designated patient age groups: Adult (18-65 years) and Geriatric (65 years and up).

Demonstrates knowledge of and follows departmental and hospital guidelines related to customer service activities in the performance of the duties and responsibilities of the position.
Demonstrates awareness of patients needs as a total person (physical, spirtual, emotional, emotional, intellectual, cultural, etc.) consistent with patient's age development level.
Adheres to and follows Workplace Ethics, Compliance Program Standards and policies & procedures.
Has working knowledge of the hospital fire, disaster, and infection control plan
Follows a regime for professisonal growth.

Works collaboratively with all members of the Stroke Program, Stroke Director, Manager of Accreditation & Licensure, Director of Quality Management.
Performs administrative duties and services related to quality, safety, education and policy & procedure.
Incorporates the knowledge of American Heart Association as well as emerging science and evidence based practice into the Stroke Program standards.
Participates in Acute Stroke Code activations when available, to ensure protocols are followed.
Monitors the operational efficiency of the Stroke Program.
Promotes marketing efforts for the Stroke Program to internal and external audiences that will support the Program image to the community.

Participates in organizational effectiveness activities; serving as co-chair of the monthly Stroke Meeting, and as member on hospital, network, and/or professional committees.
Performs concurrent and retrospective chart review to ensure compliance with Clinical Practice Guidelines and Quality Measures.
Maintains Primary Stroke Center certification, through knowledge of current standards.
Facilitate best practice based on relevant Clinical Practice Guidelines
Collect and analyze stroke alert data.
Performs root cause analysis and direct feedback to nursing leadership for nursing related fall out cases.
Initiates and leads stroke program Performance Improvement projects as results of data analysis.

Facilitates daily interdisciplinary rounding on all stroke patients, ensuring a smooth transition rounding on all stroke patients, ensuring a smooth transition of care from one level to the next ensuring quality stroke care.
Provides advanced practice nursing consultation on complex stroke patients, functioning as a resource for patients, family and nursing staff.
Provides acute stroke assessment through attending stroke alerts as appropriate.
Performs in the role of a study coordinator for the implementation and monitoring of assigned stroke specific clinical research activities. Actively recruits, screens, enrolls, and follows all patient entered into stroke clinical research.

Staff Education
Designs and implements stroke education across all health care disciplines in collaboration with nursing and other departments, ensuring certification requirements are met.
Supports development of nursing staff through real experience coaching of assessment, patient education, and core metrics.

Community Outreach
Participates in design and delivery of community education and outreach.

Has working knowledge of the hospital fire, disaster, and infection control plan

Minimum of 1 year experience with care of neurovascular patients is preferred.
Critical care experience preferred.
Knowledge of research principles.
Exhibits good organization, communication, and planning skills.
Demonstrates good leadership, decision - making and judgment skills
Communicates, advises, writes and consults in a professional manner.
Experience with Joint Commission Certification process preferred
Current New York State License (Physician Assistant of Nurse Practitioner)

 Experience working with stroke patients

  • Proof of Covid-19 vaccination is required prior to hire
